First thing: Submit 2 Forms
1. Fill out for a USPTO 'Customer Number'
2. Fill out a 'Certificate Action Form' to prove your ID (It must be notarized)
I was told by the USPTO that the most direct way to apply is to mail in both forms together to the same address at the same time.
